This is an update to two separate posts from last spring about my prostate cancer. Despite PSA scores that continued to be lower than the one that lead to the previous biopsies, my most recent biopsy revealed two additional tumors that like the first one are also 6 on the Gleason scale. The tumors are small, but since there are more of them it might be wise to consider treatment. My cancer is still stage one, and since I'm also slim I might be a candidate for nerve sparing robotic surgery (for which only stage one patients with BMIs of <25 whose tumors are not on the surface of the prostate qualify) from which odds are excellent that I would recover with full control of my bladder (after 3-6 months of post-operative incontinence) and no loss of sexual potency (though it will take between 6-12 months post-surgery to get that part of my anatomy working to its current capability). I'll have an MRI at the end of the month to determine whether any of the three tumors are on the surface of my prostate; if none of them are I'll have the surgery in early February. 

 

If on the other hand the MRI reveals a surface location of any of the tumors I'll revert to active surveillance in the short run and consider other treatments in the intermediate run, including some that look promising but will not be available for a few years. Since I'm stage one there is no urgency to beginning treatment. I'm only considering surgery now because robotic surgery has excellent outcomes for stage one patients, especially younger sexually virile patients with no prior history of incontinence (the average age of patients is 65--nine years older than me). Meanwhile I'm keeping a positive attitude and increasing my exercise regimen to prepare for surgery should my MRI allow me to proceed.

I took my last Cipro today and saw my urologist. During the exam my genitals were still quite sensitive to his touch; he said Shoshana and I should wait another 24 hours before resuming conjugal relations. He explained that the epididymis has numerous microscopic channels in which bacteria like to hang out, and I should not be surprised if I experience another relapse (in which event I should email him immediately at any hour, and he will get back to me on his blackberry). His parting Hanukah gifts to me were undated RXs for antibiotics and reminded me to bring some along when Shoshana and I travel to Jacksonville at the end of next month.

On Friday some of the symptoms I experienced during my October infection reoccurred, and I've been back on Cipro since Friday night. The symptoms are diminished somewhat, and I'm waiting for my urologist to reply to my email. When I saw him three weeks ago he told me that this is now a vulnerable area, and he would not be surprised if the infection returned. I didn't think it would be back this soon.

Saturday evening we dined out in the neighborhood with the in-laws, and upon returning I took no more than eight steps back into the apartment when I unexpectedly vomited--not pretty. It's weird that I had no warning and could not get to the toilet in time. We think it must be a food allergy to the salad greens that had a similar effect a few years ago. Back then I thought it was the fact that organic greens are fertilized with manure; now a food allergy seems the more likely culprit. I wish I knew to which green in particular I'm allergic.

1) Call plumber about faulty heating zone and thermostat.

2) Shop for travel (airfare, hotel & car rental) for late January/early February trip to Jacksonville.

3) Start practicing Torah trope for 01/31/09.

4) Put candy near front door for trick or treaters.

5) Try to continue staying horizontal as much as possible for final two days on anti-biotic.

Regarding the last item, not fighting gravity has helped the antibiotic fight the infection. I've been in bed or on the couch, physically inactive (except for sex which resumed Monday--Thank God, I can be oversexed again), and I've lost weight despite feeding a voracious appetite, which must mean my body has been burning calories fighting bacteria. I've craved dairy products in general and cheese in particular, so my body probably needs extra protein. Saturday will be my final day on the anti-biotic; I'll see if I feel up to going to shul or whether I'm better off staying home and supine. On Monday I'll get a sonogram of my bladder & kidneys. I'll have to fill my bladder to the point where I'll need to pee and then hold it while they do the sonogram. What fun!

2/3 through a course of Cipro (antibiotic) my stamina is low but not as low as at first, my urine is still bloody, my scrotum is less swollen but swollen nonetheless & tender to touch, and it feels like the infection has moved into the groin area where I had hernia surgery 8 years ago. I'll see my urologist tomorrow morning. If I don't see greater improvement I may have to postpone my Chicago trip yet again.
